Im not sure of the legitimacy of this image. Seeing as 1991 is the stated date of exposure, im willing to surmise that "wrist trailer" is the most logical item if it is a verified original. Wrist training is used in fighting training to increase wrist strength, which is crucial to executing efficient pinching in boxing.

We must do this by process of elimination. Firstly, we have to date the photo. We know that Tyson dated Naomi in 1989-1990. If you look at the evolution of cellphones, you will find *nothing* of that size and shape existed before the turn of millenium. It also can't be a PalmPilot. Those, too, came later. Looking into the history of handheld electronics in general, there is only one that seems to fit the profile, and it was released in 1989, exactly when he was dating Campbell. That would be the Nintendo Game Boy. It can be either that, or one other metallic-ish, slim handheld device relevant to him, which would be a Nintendo NES Mike Tyson's Punch-Out! Perhaps he was field-testing a new release. Whichever it is, I'm the best guesser, and I rest my case.
Sorry, my English is not very good. but:
Please zoom in on the image. The index finger is open - the next two fingers hold the object - and the last finger on the back of the object balances the object. Exactly the same as when you hold your cell phone or notebook in your hand. The little finger or the last finger is very important, so zoom in on the image and stare at it carefully. The little finger - even with perspective - is higher than the other fingers, and its inclination is downward. So it can not be under the object. So as I said, the little finger is behind the object (not under it). So this object is not like a notebook or a rectangular cell phone, but a square. Yes square! You might say to yourself that today we also have a cell phone and a square notebook. So let's go a little further ...
Zoom in on the image and look at the top of the object. There is a black line or shadow on top of the object. This shadow indicates that there is a bulge at the top of the object - on the right. A bulge at the top of the object that is seen along it.
Given that:
1- The object is square
2. There is a bulge at the top that is slightly curved
3- The photo was taken in 1991
4- Mike Tyson did not travel in time! If he has traveled, why should he pick up his cell phone in a world where there is no internet or waves? Even if he had a cell phone, he should have put it in his pocket because it was not useful at that time and he certainly did not wait for a call in 1991! Just laugh...
So according to items 1 and 2, I can say that it is definitely not a smartphone.
I think it is a small mirror that belongs to the girl. Perhaps...
This photo was taken by photographer Bruce Weber for Vogue magazine and used in the article "Fight Knight" which appeared in the December 1989 issue. As mentioned, the date makes it impossible for this to be a smart phone as the only viable candidate for the time would be the Motorola MicroTAC released in April 1989. As for the possibility of a GameBoy (mentioned elsewhere), the US version was not released until July 1989 and was larger than what Mike is holding...and why on earth would he be carrying a GameBoy? This item looks like it has a metallic sheen (higher quality: https://i.pinimg.com/originals/6c/92/9a/6c929ab5cac0adc5907a09396fac01d0.jpg). There were other 'proofs' taken from this time (according to Vogue article) but they seem lost to history. We may never know the exact item, but my guess is a cigarette case or metallic card/money holder.
